    Artificial Intelligence Algorithms to Diagnose Glaucoma and Detect Glaucoma Progression: Translation to Clinical Practice

    Purpose: This concise review aims to explore the potential for the clinical implementation of artificial intelligence (AI) strategies for detecting glaucoma and monitoring glaucoma progression. / Methods: Nonsystematic literature review using the search combinations "Artificial Intelligence," "Deep Learning," "Machine Learning," "Neural Networks," "Bayesian Networks," "Glaucoma Diagnosis," and "Glaucoma Progression." Information on sensitivity and specificity regarding glaucoma diagnosis and progression analysis as well as methodological details were extracted. / Results: Numerous AI strategies provide promising levels of specificity and sensitivity for structural (e.g. optical coherence tomography [OCT] imaging, fundus photography) and functional (visual field [VF] testing) test modalities used for the detection of glaucoma. Area under receiver operating curve (AROC) values of > 0.90 were achieved with every modality. Combining structural and functional inputs has been shown to even more improve the diagnostic ability. Regarding glaucoma progression, AI strategies can detect progression earlier than conventional methods or potentially from one single VF test. / Conclusions: AI algorithms applied to fundus photographs for screening purposes may provide good results using a simple and widely accessible test. However, for patients who are likely to have glaucoma more sophisticated methods should be used including data from OCT and perimetry. Outputs may serve as an adjunct to assist clinical decision making, whereas also enhancing the efficiency, productivity, and quality of the delivery of glaucoma care. Patients with diagnosed glaucoma may benefit from future algorithms to evaluate their risk of progression. Challenges are yet to be overcome, including the external validity of AI strategies, a move from a "black box" toward "explainable AI," and likely regulatory hurdles. However, it is clear that AI can enhance the role of specialist clinicians and will inevitably shape the future of the delivery of glaucoma care to the next generation. / Translational Relevance: The promising levels of diagnostic accuracy reported by AI strategies across the modalities used in clinical practice for glaucoma detection can pave the way for the development of reliable models appropriate for their translation into clinical practice. Future incorporation of AI into healthcare models may help address the current limitations of access and timely management of patients with glaucoma across the world

    Change in level of productivity in the treatment of schizophrenia with olanzapine or other antipsychotics

    <p>Abstract</p> <p>Background</p> <p>When treating schizophrenia, improving patients' productivity level is a major goal considering schizophrenia is a leading cause of functional disability. Productivity level has been identified as the most preferred treatment outcome by patients with schizophrenia. However, little has been done to systematically investigate productivity levels in schizophrenia. We set out to better understand the change in productivity level among chronically ill patients with schizophrenia treated with olanzapine compared with other antipsychotic medications. We also assessed the links between productivity level and other clinical outcomes.</p> <p>Methods</p> <p>This post hoc analysis used data from 6 randomized, double-blind clinical trials of patients with schizophrenia or schizoaffective disorder, with each trial being of approximately 6 months duration. Change in productivity level was compared between olanzapine-treated patients (HGBG, n = 172; HGHJ, n = 277; HGJB, n = 171; HGLB, n = 281; HGGN, n = 159; HGDH, n = 131) and patients treated with other antipsychotic medications (separately vs. haloperidol [HGGN, n = 97; HGDH, n = 132], risperidone [HGBG, n = 167; HGGN, n = 158], quetiapine [HGJB, n = 175], ziprasidone [HGHJ, n = 271] and aripiprazole [HGLB, n = 285]). Productivity was defined as functional activities/work including working for pay, studying, housekeeping and volunteer work. Productivity level in the prior 3 months was assessed on a 5-point scale ranging from no useful functioning to functional activity/work 75% to 100% of the time.</p> <p>Results</p> <p>Chronically ill patients treated with olanzapine (OLZ) experienced significantly greater improvement in productivity when compared to patients treated with risperidone (RISP) (OLZ = 0.22 ยฑ 1.19, RISP = -0.03 ยฑ 1.17, p = 0.033) or ziprasidone (ZIP) (OLZ = 0.50 ยฑ 1.38, ZIP = 0.25 ยฑ 1.27, p = 0.026), but did not significantly differ from the quetiapine, aripiprazole or haloperidol treatment groups. Among first episode patients, OLZ therapy was associated with greater improvements in productivity levels compared to haloperidol (HAL), during the acute phase (OLZ = -0.31 ยฑ 1.59, HAL = -0.69 ยฑ 1.56, p = 0.011) and over the long-term (OLZ = 0.10 ยฑ 1.50, HAL = -0.32 ยฑ 1.91, p = 0.008). Significantly more chronically ill and first episode patients treated with olanzapine showed moderately high (>50%-75% of the time) and high levels of productivity (>75%-100% of the time) at endpoint, when compared to risperidone or haloperidol-treated patients (p < .05), respectively. Higher productivity level was associated with significantly higher study completion rates and better scores on the positive, negative, disorganized thoughts, hostility and depression subscales of the Positive and Negative Symptom Scale (PANSS).</p> <p>Conclusions</p> <p>Some antipsychotic medications significantly differed in beneficial impact on productivity level in the long-term treatment of patients with schizophrenia.     Global, regional, and national prevalence and mortality burden of sickle cell disease, 2000-2021: a systematic analysis from the Global Burden of Disease Study 2021

    BACKGROUND: Previous global analyses, with known underdiagnosis and single cause per death attribution systems, provide only a small insight into the suspected high population health effect of sickle cell disease. Completed as part of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021, this study delivers a comprehensive global assessment of prevalence of sickle cell disease and mortality burden by age and sex for 204 countries and territories from 2000 to 2021. METHODS: We estimated cause-specific sickle cell disease mortality using standardised GBD approaches, in which each death is assigned to a single underlying cause, to estimate mortality rates from the International Classification of Diseases (ICD)-coded vital registration, surveillance, and verbal autopsy data. In parallel, our goal was to estimate a more accurate account of sickle cell disease health burden using four types of epidemiological data on sickle cell disease: birth incidence, age-specific prevalence, with-condition mortality (total deaths), and excess mortality (excess deaths). Systematic reviews, supplemented with ICD-coded hospital discharge and insurance claims data, informed this modelling approach. We employed DisMod-MR 2.1 to triangulate between these measures-borrowing strength from predictive covariates and across age, time, and geography-and generated internally consistent estimates of incidence, prevalence, and mortality for three distinct genotypes of sickle cell disease: homozygous sickle cell disease and severe sickle cell ฮฒ-thalassaemia, sickle-haemoglobin C disease, and mild sickle cell ฮฒ-thalassaemia. Summing the three models yielded final estimates of incidence at birth, prevalence by age and sex, and total sickle cell disease mortality, the latter of which was compared directly against cause-specific mortality estimates to evaluate differences in mortality burden assessment and implications for the Sustainable Development Goals (SDGs). FINDINGS: Between 2000 and 2021, national incidence rates of sickle cell disease were relatively stable, but total births of babies with sickle cell disease increased globally by 13ยท7% (95% uncertainty interval 11ยท1-16ยท5), to 515โ€‰000 (425โ€‰000-614โ€‰000), primarily due to population growth in the Caribbean and western and central sub-Saharan Africa. The number of people living with sickle cell disease globally increased by 41ยท4% (38ยท3-44ยท9), from 5ยท46 million (4ยท62-6ยท45) in 2000 to 7ยท74 million (6ยท51-9ยท2) in 2021. We estimated 34โ€‰400 (25โ€‰000-45โ€‰200) cause-specific all-age deaths globally in 2021, but total sickle cell disease mortality burden was nearly 11-times higher at 376โ€‰000 (303โ€‰000-467โ€‰000). In children younger than 5 years, there were 81โ€‰100 (58โ€‰800-108โ€‰000) deaths, ranking total sickle cell disease mortality as 12th (compared to 40th for cause-specific sickle cell disease mortality) across all causes estimated by the GBD in 2021. INTERPRETATION: Our findings show a strikingly high contribution of sickle cell disease to all-cause mortality that is not apparent when each death is assigned to only a single cause. Sickle cell disease mortality burden is highest in children, especially in countries with the greatest under-5 mortality rates. Without comprehensive strategies to address morbidity and mortality associated with sickle cell disease, attainment of SDG 3.1, 3.2, and 3.4 is uncertain. Widespread data gaps and correspondingly high uncertainty in the estimates highlight the urgent need for routine and sustained surveillance efforts, further research to assess the contribution of conditions associated with sickle cell disease, and widespread deployment of evidence-based prevention and treatment for those with sickle cell disease.     Global, regional, and national burden of stroke and its risk factors, 1990-2019: a systematic analysis for the Global Burden of Disease Study 2019

    Background: Regularly updated data on stroke and its pathological types, including data on their incidence, prevalence, mortality, disability, risk factors, and epidemiological trends, are important for evidence-based stroke care planning and resource allocation. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) aims to provide a standardised and comprehensive measurement of these metrics at global, regional, and national levels. Methods: We applied GBD 2019 analytical tools to calculate stroke incidence, prevalence, mortality, disability-adjusted life-years (DALYs), and the population attributable fraction (PAF) of DALYs (with corresponding 95% uncertainty intervals [UIs]) associated with 19 risk factors, for 204 countries and territories from 1990 to 2019. These estimates were provided for ischaemic stroke, intracerebral haemorrhage, subarachnoid haemorrhage, and all strokes combined, and stratified by sex, age group, and World Bank country income level. Findings: In 2019, there were 12ยท2 million (95% UI 11ยท0โ€“13ยท6) incident cases of stroke, 101 million (93ยท2โ€“111) prevalent cases of stroke, 143 million (133โ€“153) DALYs due to stroke, and 6ยท55 million (6ยท00โ€“7ยท02) deaths from stroke. Globally, stroke remained the second-leading cause of death (11ยท6% [10ยท8โ€“12ยท2] of total deaths) and the third-leading cause of death and disability combined (5ยท7% [5ยท1โ€“6ยท2] of total DALYs) in 2019. From 1990 to 2019, the absolute number of incident strokes increased by 70ยท0% (67ยท0โ€“73ยท0), prevalent strokes increased by 85ยท0% (83ยท0โ€“88ยท0), deaths from stroke increased by 43ยท0% (31ยท0โ€“55ยท0), and DALYs due to stroke increased by 32ยท0% (22ยท0โ€“42ยท0). During the same period, age-standardised rates of stroke incidence decreased by 17ยท0% (15ยท0โ€“18ยท0), mortality decreased by 36ยท0% (31ยท0โ€“42ยท0), prevalence decreased by 6ยท0% (5ยท0โ€“7ยท0), and DALYs decreased by 36ยท0% (31ยท0โ€“42ยท0). However, among people younger than 70 years, prevalence rates increased by 22ยท0% (21ยท0โ€“24ยท0) and incidence rates increased by 15ยท0% (12ยท0โ€“18ยท0). In 2019, the age-standardised stroke-related mortality rate was 3ยท6 (3ยท5โ€“3ยท8) times higher in the World Bank low-income group than in the World Bank high-income group, and the age-standardised stroke-related DALY rate was 3ยท7 (3ยท5โ€“3ยท9) times higher in the low-income group than the high-income group. Ischaemic stroke constituted 62ยท4% of all incident strokes in 2019 (7ยท63 million [6ยท57โ€“8ยท96]), while intracerebral haemorrhage constituted 27ยท9% (3ยท41 million [2ยท97โ€“3ยท91]) and subarachnoid haemorrhage constituted 9ยท7% (1ยท18 million [1ยท01โ€“1ยท39]). In 2019, the five leading risk factors for stroke were high systolic blood pressure (contributing to 79ยท6 million [67ยท7โ€“90ยท8] DALYs or 55ยท5% [48ยท2โ€“62ยท0] of total stroke DALYs), high body-mass index (34ยท9 million [22ยท3โ€“48ยท6] DALYs or 24ยท3% [15ยท7โ€“33ยท2]), high fasting plasma glucose (28ยท9 million [19ยท8โ€“41ยท5] DALYs or 20ยท2% [13ยท8โ€“29ยท1]), ambient particulate matter pollution (28ยท7 million [23ยท4โ€“33ยท4] DALYs or 20ยท1% [16ยท6โ€“23ยท0]), and smoking (25ยท3 million [22ยท6โ€“28ยท2] DALYs or 17ยท6% [16ยท4โ€“19ยท0]). Interpretation: The annual number of strokes and deaths due to stroke increased substantially from 1990 to 2019, despite substantial reductions in age-standardised rates, particularly among people older than 70 years. The highest age-standardised stroke-related mortality and DALY rates were in the World Bank low-income group. The fastest-growing risk factor for stroke between 1990 and 2019 was high body-mass index. Without urgent implementation of effective primary prevention strategies, the stroke burden will probably continue to grow across the world, particularly in low-income countries.
